Whether you choose self-pay or use insurance through participating plans as they become available, my goal is to make financial aspects of care straightforward and easy to understand.

New Patient Evaluation
Initial Psychiatric Evaluation (60 minutes)

A comprehensive psychiatric evaluation that includes a thorough assessment of your symptoms, medical and mental health history, and current concerns. The visit includes an individualized treatment plan, evidence-based recommendations, and clear treatment goals focused on lasting improvement.
Follow-Up Care
Medication Management & Supportive Therapy (Up to 30 minutes)
$175
A personalized follow-up appointment that includes medication management, review of symptoms and treatment progress, supportive psychotherapy as clinically appropriate, and thoughtful adjustments to your treatment plan to support lasting wellness.
Appointments are intentionally scheduled to provide adequate time to review symptoms, assess treatment response, address questions, and support meaningful progress, not simply renew prescriptions.
Longer follow-up appointments may occasionally be recommended when additional assessment or treatment planning is needed. This will always be discussed in advance.
Payment & Insurance
Root & Rise Brain Health accepts self-pay patients and participates with select insurance plans through Headway.
California: Aetna, Cigna, participating Carelon Behavioral Health plans, and Anthem Blue Cross of California.
Arizona: Aetna, Cigna, participating Carelon Behavioral health plans, and Curative.
Additional Arizona plans are currently in credentialing, including Blue Cross Blue Shield of Arizona.
Patients who wish to use insurance must verify their individual eligibility, benefits, and estimated costs through Headway. Participation in an insurance network does not guarantee that every plan or service is covered.
A valid payment method is required to schedule and maintain appointments. Self-pay appointment fees may be charged to the payment method on file up to 24 hours before the scheduled appointment.
Superbills are available upon request. A superbill may be submitted by the patient to their insurance plan for possible out-of-network reimbursement. Root & Rise Brain Health does not submit out-of-network claims on the patient’s behalf. Reimbursement is determined solely by the patient’s insurance plan and cannot be guaranteed.
Administrative & Clinical Forms
Requests for forms, letters, workplace accommodations, leave documentation, or other administrative paperwork are considered on a case-by-case basis and must be clinically appropriate and supported by the medical record. Completion of requested documentation is not guaranteed. Additional fees may apply depending on the complexity and time required.
Payment and Cancellation Policy
A valid payment method is required to schedule and maintain appointments. Self-pay appointment fees may be charged up to 24 hours before the scheduled appointment.
Appointments must be canceled or rescheduled at least 48 hours in advance. Late cancellations and missed appointments are subject to a $150 fee for a new patient psychiatric evaluation and a $75 fee for follow-up appointments. These fees are not billed to insurance. Exceptions may be considered for emergencies or extenuating circumstances at the practice’s discretion.
Removing a payment method does not eliminate responsibility for services or fees already incurred. If a payment method is declined or unavailable, routine appointments may be postponed until payment arrangements are completed.
Uninsured and self-pay patients will receive a Good Faith Estimate when required under federal law.
Complete financial terms and payment authorization are provided during intake and must be reviewed and signed before treatment begins.
