Patients are responsible for accurately reporting their physical location at the time of a telehealth encounter.
EMERGENCIES AND URGENT CONCERNS
The Root & Rise Brain Health website, contact forms, email, scheduling systems, and patient portal are not emergency services and should not be used for emergency communication.
If you are experiencing a medical or psychiatric emergency, believe you may harm yourself or another person, or require immediate assistance, call 911 or go to the nearest emergency department.

Root & Rise Brain Health provides psychiatric services to adults age 18 and older.

PRESCRIBING AND CONTROLLED MEDICATIONS
